Creating a Healthier, Happier Community

Our Access to Health Care program employs community health workers to help individuals navigate the health care system, overcome barriers to care and connect with a primary medical home. Research shows when individuals use a primary medical home they are healthier and less reliant on emergency rooms for non-urgent issues.

Access to Health Care Program Results

968 People

now have medical insurance.

2,412 People

now have a primary medical home.

4,385 People

have been referred to services like free medical clinics and
food banks.
