Creating a Healthier, Happier Community
Our Contact to Care program is built to help people connect with health care providers in the area. The project employs community health workers to help individuals navigate the health care system, overcome barriers to care and connect with a primary medical home. Research shows when individuals use a primary medical home they are healthier and less reliant on emergency rooms for non-urgent issues.
