We launched this community initiative to increase access to health care for those in our community. We’ve added dental, vision, and behavioral health services since the program launched. See how Contact to Care is changing lives right here in the Capital Region.

Creating a Healthier, Happier Community

Our Contact to Care program is built to help people connect with health care providers in the area. The program employs community health workers to help individuals navigate the health care system, overcome barriers to care and connect with a primary medical home. Research shows when individuals use a primary medical home they are healthier and less reliant on emergency rooms for non-urgent issues.

Contact to Care Results

901 People

now have medical insurance.

2,123 People

now have a primary medical home.

3,817 People

have been referred to services like free medical clinics and
food banks.
