We launched this community pilot project to increase access to health care for those in our community. We’ve added dental, vision, and behavioral health services since the program launched.

Creating a Healthier, Happier Community

Our Contact to Care program is built to help people connect with health care providers in the area. The project employs community health workers to help individuals navigate the health care system, overcome barriers to care and connect with a primary medical home. Research shows when individuals use a primary medical home they are healthier and less reliant on emergency rooms for non-urgent issues.

now have medical insurance.

654 People

now have a primary medical home.

1,753 People

have been referred to free medical clinics and other support services.
